HOW TO APPLY FOR A MEDICAL CANNABIDIOL REGISTRATION CARD:
WHAT IS A MEDICAL CANNABIDIOL REGISTRATION CARD?
Cannabidiol Registration Cards provide documentation that patients and/or caregivers are authorized to legally possess medically necessary cannabidiol. The Medical Cannabidiol Registration Cards look very similar to an Iowa Driver's License and includes your personal information including name, address, date of birth, registration number, etc. In order to get a card, the Iowa Department of Health and the Iowa Department of Transportation approve and generate the cards. It allows both patients and caregivers who complete the Iowa Department of Health's application process are approved to legally possess medical cannabidiol (in accordance with HF524).
HOW DO I APPLY?
In order to enroll in the program, patients* must obtain a written certification from their doctor (who must be a board certified physician) via the application on the Iowa Department of Health's website. In addition, the IDPH also has patient information sheets to help you discuss cannabidiol with your doctor. Applications can be found here. The fee for each patient is $100.
Once your application is filled out by both you and your physician, you will receive a letter in the mail from the Iowa Department of Health. If approved, you simply take that letter to the Iowa Department of Transportation's Iowa Driver's Licensure Issuance Site to get your photo taken and have your card issues.
SUGGESTIONS FROM OTHER PATIENTS/CAREGIVERS:
Caregivers and patients have suggested having the entire application from the Iowa Department of Health completed for your physician before you meet with them. Go through the checklist that comes with the application and make sure that you have all of your needed documentation copied and included with your application before you even approach your physician. Do NOT call your doctor to discuss this topic. This is a discussion that is to be made - in person - with a doctor that you know and trust.
